Bahama Splash Web Registration Form

Use this on-line form to reserve seats for the Bahama Splash event on Saturday, April 17, 2010. Download the flyer for event details. You may also use this form to donate to our Scholarship Fund.

This event is open to CCNY alumni, family and friends. Please respond by April 10, 2010. Reservations are made only when payment is received on a first come, first served basis. Some event seating is limited.

Instructions:

Step 1: Get your checkbook, then fill out the form below. Items marked * are required. Make your check out to CCNY-SNC.
Step 2: Click Submit to start your reservation process. An acknowledgement page appears. Print it to show you registered.
            (Note: if you get a CGI error message, temporarily disable your firewall, then resubmit)
Step 3: Mail your check for $39.50 per person to:

Your privacy is important. We need to contact you in case there is a change beyond our control or if payment has been delayed. The information about you that you provide:

  1. Is not shared with third parties.
  2. Is limited to your name, address, phone number, and e-mail address (contact information).
  3. Is for internal Chapter use in order to confirm your reservation and contact you if there is a problem.
  4. Is not sent to us on a secure link, so can be read by anyone with the proper software. However, the limited contact information that we request is readily available from many other sources (e.g., telephone directory, CCNY Alumni Association, existing mail or phone lists, etc).

Note: We do not accept credit cards. Nobody calling you from the Southern Nevada Chapter will ever ask you for a credit card number, social security number, account number, or any other personal financial information.
